Consideraciones especiales en la planificación quirúrgica de procedimientos electivos y de emergencia en tiempos de pandemia por COVID-19.
Special considerations in surgical planning of elective and emergency procedures in times of COVID-19 pandemic

Tras el inicio de la pandemia en marzo de 2020, alrededor de 3,5 millones de personas han muerto a causa del COVID-19, afectando la operatividad en todos los sistemas de salud. En consecuencia, se ha intentado llevar a un segundo plano la atención quirúrgica electiva, incluso diferirse, tratándose de un tema crítico, pues equilibrar el beneficio quirúrgico con el riesgo desconocido a contraer COVID-19 y sus complicaciones en pacientes, y personal médico, permanece latente. Sin embargo, las cirugías deben practicarse eventualmente por comprometer pronóstico y funcionalidad de algunos individuos, lo cual ha permitido recuperar el número de cirugías electivas bajo condiciones especiales, siendo un factor clave en la administración sanitaria. La presente revisión resalta la necesidad de establecer prácticas y/o protocolos clínicos seguros para prevenir contagios por COVID-19 en quirófano. Para ello, se realizó la búsqueda en repositorios virtuales, PubMed y Google Scholar utilizando descriptores sobre el tema en artículos científicos y páginas web institucionales, evidenciándose actualizaciones sobre medidas de protección para el manejo perioperatorio del paciente, destacando que la detección de COVID-19 mediante pruebas diagnóstico, es esencial para proteger a este y al personal médico-quirúrgico del riesgo infeccioso, especialmente durante el período intraoperatorio. Asimismo, el aislamiento obligatorio de pacientes con criterio quirúrgico positivos para COVID-19 o aquellos con sintomatología sospechosa para el virus, minimizando la propagación del patógeno durante su hospitalización.

After the beginning of the pandemic in March 2020, around 3.5 million people have died from COVID-19, affecting the operation of all health systems. Consequently, an attempt has been made to take elective surgical care to the background, even its deferral, being this a critical issue, since the surgical benefit is balanced with the unknown risk of contracting COVID-19 and its complications in patients, and medical personnel, remains latent. However, surgeries must eventually be performed due to compromising prognosis and functionality of some individuals, which has made it possible to recover the number of elective surgeries under special conditions, being this a key factor in health administration. This review highlights the need to establish safe clinical practices and / or protocols to prevent COVID-19 infections in the operating room. For doing this, a research was carried out in virtual repositories, PubMed and Google Scholar using descriptors on the subject in scientific articles and institutional web pages, evidencing updates on protection measures for perioperative patient management, highlighting that detection of COVID-19 through diagnostic tests are essential to protect the patient and the medical-surgical personnel from infectious risk, especially during the intraoperative period. Likewise, the mandatory isolation of patients with positive surgical criteria for COVID-19 or those with suspicious symptoms for the virus, minimizing the spread of the pathogen during their hospitalization time.
