Desafíos actuales de la restauración post-endodoncia. Revisión narrativa

Resumen


La rehabilitación de dientes tratados endodónticamente con daño extenso, sigue siendo un desafío. El uso de restauraciones con poste se ha cuestionado debido al posible debilitamiento dental. En esta revisión narrativa, se evaluó y comparó el desempeño biomecánico de endocoronas y coronas-muñón
tradicionales (con y sin poste intraconducto) y restauraciones directas/fibras, para la rehabilitación de dientes tratados endodónticamente con daño severo de la estructura coronal. La literatura analizada abarcó publicaciones hasta marzo 2025. Se realizó una búsqueda de estudios in vitro y clínicos en inglés en tres bases de datos de investigación (PubMed, Google Scholar, and SciELO), complementada con una exploración manual en las bibliografías de los estudios encontrados. Se excluyeron los informes de casos. Las endocoronas son una alternativa fiable a las restauraciones post-retenidas, pero se debe respetar un diseño de preparación específico y un protocolo de adhesión riguroso. Cuando se debe elegir una restauración de bajo costo, la base de SFRC mejorará significativamente el rendimiento y el modo de falla de las restauraciones directas. Entre los materiales disponibles para técnicas indirectas, destacan los procedimientos CAD/CAM, Cerámica vítrea de disilicato de litio, Cerámica infiltrada con polímero y el Silicato de litio reforzado con zirconio, sin embargo, se debe tener precaución en la selección del material, ya que afecta la resistencia a la fatiga y el modo de falla. Esta revisión muestra que las endocoronas presentan un rendimiento biomecánico similar o superior al de las restauraciones con poste-corona.
